Code Assignment

Code assignment should be driven by four primary factors (in the following order):

1. Physician documentation
2. Application of official coding guidelines to the physician’s documentation of the patient’s condition
3. Application of hospital specific guidelines in the absence of official guidelines to address a specific coding dilemma
4. Query the physician when clinically appropriate.
