This Sequential Organ Failure Assessment (SOFA) Score chart is a validated tool to quantify the level of organ dysfunction in critically ill patients and estimate mortality risk. It can also be used as a criterion for diagnosing sepsis in the presence of suspected infection per Sepsis-3 criteria.
